ATI Real Life Clinical Scenario: Schizophrenia

Please answer the following questions:  Identify priority assessments for patients who have schizophrenia, especially the behavioral characteristics common to the disorder. o Assess for command hallucinations o Assess for trouble swallowing o Assess for dizziness o Use the Suicide Assessment Five Step Evaluation and Triage (SAFE-T)  List several priority nursing diagnoses for the patient who has schizophrenia. o Disturbed sensory perception r/t auditory hallucinations o Impaired social interaction r/t paranoia o Impaired verbal communication r/t hallucination  Recognize potential ethical issues that can arise with adolescent patients in acute crisis.  Identify important nursing considerations for medications used to treat schizophrenia. What are some of those medications? o Risperidone o Paliperidone  Risk of neuropoietic malignant syndrome  Complete an accurate and effective SBAR communication tool. o S: Presents to the outpatient clinic today for scheduled appointment to assess advancement of schizophrenia diagnosis

o B: 21 year-old male with a history of schizophrenia. Was prescribed risperidone but has not taken the medication in the last 4 weeks because he is afraid the pharmacist is poisoning the medications. o A: Anxious, restless, auditory hallucinations delusional ideation o R: putting the patient on an injectable medication instead of oral medication so that the idea that the pharmacist is trying to poison the pills.

Please answer the following questions:  What would you expect to find on a Mental Status Exam on a patient who has a mood disorder? Use descriptive words related to Mood Disorders. (See your textbook) o Rapid cycling o May neglect self-care or may dress in colorful clothing with excessive make up o Pressured speech o Restlessness, agitation, psychomotor agitation o Labile mood and affect o Hallucinations o Loose associations, flight of ideas o Awake, alert, lethargic or stupors o Impairment of short- and long-term memory  Develop a plan of care for a patient who has a mood disorder.

 Complete an accurate and effective SBAR communication tool. o S: Patient admitted by PCP to the inpatient psych unit due to signs of manic episode. Patient is alert and responsive o B: 33-year-old female. Has been being treated for depression with Fluoxetine. Presents today with a manic episode. o A: Dressed in colorful clothing and makeup, pressured speech, psychomotor agitation, euphoric mood, labile affect, loose association. o R: needs lithium therapy for stabilization of mood and individual therapy to work on effective coping skills.
