Anticoagulant Therapy
Renal Function
Therapeutic INR: 2.0-3.0
PT: 11-12.5 seconds. Therapeutic range for

PTT: 30-40 seconds. Therapeutic range for
anticoagulant therapy is 1.5 to 2 times the
normal or control value
INR: Normal INR is 0.8-1.1; Critical value >5
Platelets: 150,000-400,000 /mm3
Serum creatinine:
Males 0.6-1.2 mg/dL
Females 0.5-1.1 mg/dL
BUN: 10-20 mg/dL
Creatinine clearance test:
Males 90-139 mL/min
Females 80-125 mL/min
Blood Glucose Levels
Therapeutic Medication Monitoring
Glucose (fasting): 70-105 mg/dL
Glycosylated Hemoglobin (HbA1c):
4%-6% is normal
>8% indicates poor DM control
Digoxin Level: 0.8-2.0 ng/mL
Lithium Level: 0.4-1.2 mEq/L
Phenobarbital: 10-40 mcg/mL
Theophylline: 10-20 mcg/mL
