PSYCHIATRIC NURSING SBAR TOOL: PSYCH
P : Patient
Patient Name: Louise Age 20 Diagnosis: Eating Disorder
S : SITUATION
What brought patient to hospital (brief/concise description of behavior and
circumstances):
Dehydration and then she was switched from Med/surg to psych bc we found she has
an easting disorder and altered body image.
Y : YOUR ASSESSMENT
Mental status; Problems; Concerns
Pt has a very distorted body image and when I tried to draw her body out compared to
what she has drawn, she was very upset and needed to change things. First we should
treat her dehydration and then treat with antidepressants, antianxiety, and possibly a
medication to stimulate huger. She will also need therapy for how severe her thought
processes are. We should hide all scales so she cannot see her weight when we weigh
her everyday. She should have a private room and have supervised meals and be
supervised at least an hour after meals to make sure she is not throwing up.
C : CRITICAL INFORMATION
Last medications given within last 12 hrs. Antidepressant (sertraline), Fluids (NSS 0.9%)
Allergies: NKDA
Recent CIWA score (if pertinent): N/A
Level of Agitation: 0
H : HELP
What do you need for pt. now/orders?
I think patient would benefit from:
One on one therapy and the use of a journal. She could also use a prescription for anxiety medications to
calm her down before eating her supervised meals with us.
