Opening Questions How did the simulated experience of Olivia Jones’s case make you feel?

This simulation made me feel intrigued because of the new features on the sim that I have never seen before, so I was excited to learn during this simulation.

Describe the actions you felt went well in this scenario.

Actions that I felt went well during this scenario were making sure I did all the assessment steps and incorporating therapeutic communication. Educating the patient is also a big step in the simulation.

Scenario Analysis Questions * EBP What are the indications that Olivia Jones’s preeclampsia has progressed?

Indications that Olivia’s preeclampsia has progressed includes headache, visual changes, elevated blood pressure, and edema.

PCC What should the priority teaching for Olivia Jones include?

Teaching for Olivia Jones should include signs and symptoms to report, adverse effects of magnesium sulfate, and management of diet.

PCC/I Olivia Jones was placed on magnesium sulfate for treatment of her preeclampsia. What are the indications for this drug, and what should be taught to the patient regarding side effects?

Magnesium sulfate is used to prevent seizures. Olivia should be taught about side effects and signs of toxicity. Olivia will also be taught about the importance of having her reflexes checked.

T&C What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format.

S- Olivia Jones is a 23 year old female who came to the hospital with mild preeclampsia

B- Olivia Jones is at 36 weeks gestation, BP was 146/92.

A- BP was 171/102 on assessment, patient feels nauseated and states that she is experiencing visual changes and headache. FHR-167.

R- monitor diet and be aware of worsening symptoms

S/QI Based on your experience with Olivia Jones’s case, reflect on possible nursing actions for enhanced safety and quality improvement.

Some nursing actions that I could improve on include making sure I have another nurse check the dosages, reassessing the blood pressure and respirations, and monitoring the urine output. Nursing actions for enhanced safety would include making sure the patient has a patent airway, padding the bedrails, and dimming the lights.

Concluding Questions Reflecting on Olivia Jones’s case, were there any actions you would do differently? Explain.

There are not any actions that I would do differently, because by retaking the vsim I got to learn more about how to assess a patient experiencing preeclampsia. By reading the results of my first try I was able to gain knowledge and do better on the next try.

Describe how you would apply the knowledge and skills that you obtained in Olivia Jones’s case to an actual patient care situation.

I would apply the knowledge and skills that I obtained by using therapeutic communication, being aware of abnormalities, and being there to support the patient and make them feel comfortable throughout the experience. Educating the patient on assessment steps is also crucial.
