Decision making- individual must be able to make an informed choice concerning his behavior Motivation- individual needs to be motivated to make a change in his behavior Behavior- individual must be able to recognize that his behavior is needs to change in order to engage healthy lifestyle Environment- individual must be able to access the needed foods, or actions in order to change his lifestyle Community Focused Health Promotion Models Most health promotion models that focus on communities share these common themes: Focus on community values and norms Legitimization of desirable behavior and environmental changes Participation of community leadership Planned change in which community members have control Assessment An assessment provides the basis for establishing a health promotion plan Individual assessment- age, health status, presence of risk factors, health preferences and values, social relationships Family assessment- risk factors, family strengths, relationships among family members, genetics Community assessment- structure of community, community census, population demographics, morbidity/mortality statistics, community resources
