What brings you into the clinic today
I've been noticing my heart seems to be beating too fast every once in a while lately, and it
started to worry me. My mom said I should make an appointment. You don't want to ignore
heart stuff, I guess.

I try to slow down and breathe, and just relax. The feeling usually goes away after awhile.

I think the first time was about a month ago.
How often are you having increased heart rate
Yeah, I'll get these episodes where my heart feels like it's racing.
How often do these episodes happen
I guess three or four times. It's been about once a week for the past month.

What do the palpitations feel like
Well, when my heart starts beating really fast, it just feels like a thumping in my chest. It
makes me anxious and uncomfortable.

I don't know if anything makes it "worse" or "better." It's just happening or not. (Clarified to if
anything makes the palpitations worse.)
Do the palpitations occur more often in the morning
I've only noticed them in the mornings, usually on the way to class. Maybe like once a week.
Does drinking caffeine make the palpitations worse
I get kinda jittery. And now that we're talking about it, maybe it could be affecting my heart
rate.
What do you do to try to help the palpitations (Clarified to How do you treat your symptoms?.)
I haven't tried anything specific, other than just trying to relax and breathe deep. (Clarified to

Have you taken any medications for the palpitations
No, I haven't tried anything.. wouldn't know what to try. It just goes away on its own after a
few minutes.
Do currently take any medications
I have a daily inhaler and a rescue inhaler for my asthma.
Are you taking any new medications
Yeah, I have a daily inhaler now that I've been using.
What type of medication is an inhaler
The daily inhaler is Flovent, and the rescue inhaler is Proventil.
What is the dose that you take
I just got the regular small dose, not extra strength or anything. I take two, though.

No, I've never had any problems with my heart before now.

Yeah, I've been pretty stressed lately.
What has your stress been related to
I just get overwhelmed when work and school stuff both get really busy at the same time. I
can handle them individually but it's stressful when I can't get any relief.

I've never been diagnosed, but I do feel anxious sometimes.
What do you typically have for breakfast
I'll usually have a muffin or pumpkin bread from the cafe near my house.
What do you typically have for lunch
Lunch is the meal I usually pick up near campus because of my schedule. I go to Subway
mostly and get a turkey sandwich or something.
What do you usually have for dinner
The usual stuff... meatloaf, chicken, casseroles, pasta... stuff like that.

I try to eat snacks that are kind of healthy like pretzels--sometimes if I'll treat myself and get
french fries, but I try not to do that too often. (Clarified to what I usually eat for a snack.)

I don't know how much caffeine is in them exactly, but I drink about four Diet Cokes a day.
Oh, and an energy drink or two in the morning.

Yeah, I started having energy drinks in the mornings before class. I don't like coffee.
Contain high levels of caffeine can increase your stress and anxiety and cause dehydration
over time.
OK, I understand.
Do you exercise
I'm pretty exhausted by the end of the day, so I don't have the energy to do any extra
exercise. But I'm on my feet all the time at work so I feel like that's a little bit of exercise most
days at least.
I understand being busy and having little free time however exercise is important for your
health and can decrease your stress.
That makes sense.
Do you drink alcohol
I do sometimes, yeah, when I go out with my friends.
When was your last drink of alcohol
Maybe two weeks ago.
Do you use tobacco
No. I think it's gross.
Do you use illicit drugs
No.
Do you have a fever

I haven't had thrown up, or even had an upset stomach, in a long time.
