Maternity Case 1: Olivia Jones

Guided Reflection Questions

Opening Questions How did the simulated experience of Olivia Jones’s case make you feel? Describe the actions you felt went well in this scenario. I felt like I could not do as much for the patient as I would have liked. Even when I handed her off, I her blood pressure still seemed quite high.

Scenario Analysis Questions* EBP What are the indications that Olivia Jones’s preeclampsia has progressed? Her BP indicated that her preeclampsia was progressing along with her blurred vision and headache. PCC What should the priority teaching for Olivia Jones include? The priority teaching should be to focus on calmness and peace as stressing will only increase her BP. PCC/I Olivia Jones was placed on magnesium sulfate for treatment of her preeclampsia. What are the indications for this drug, and what should be taught to the patient regarding side effects? When there is concern for a seizure, magnesium sulfate is indicated in a woman with preeclampsia. It may cause dizziness and confusion which should be taught to the patient. PCC/I Olivia Jones was given hydralazine. What is the indication for this medication? How often can it be given? What are the side effects? Hydralazine is used in high BP episodes and can be given 4 times a day. The side effects may include N/V, headache, and an upset stomach. T&C What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format.

  • The Scenario Analysis Ques2ons are correlated to the Quality and Safety Educa2on for Nurses (QSEN) competencies: Pa2ent-Centered Care (PCC), Teamwork and Collabora2on (T&C), Evidence-Based Prac2ce (EBP), Quality Improvement (QI), Safety (S), and Informa2cs (I). I would include her past history of preeclampsia, the meds she’s been given, updates to the orders, and what to continue doing. S What safety measures should be initiated while Olivia Jones is in the hospital? Seizure precautions should be taken such as putting up seizure pads, decreasing stimuli, and restricting the visitors allowed. S/QI Based on your experience with Olivia Jones’s case, reflect on possible nursing actions for enhanced safety and quality improvement. I would like to be particularly aware of how much BP can influence the patient’s symptoms and how to prevent seizure. EMP How would you have handled the situation if Olivia Jones had experienced a seizure? I would time it, call the provider and charge nurse.

Concluding Questions Reflecting on Olivia Jones’s case, were there any actions you would do differently? Explain. I would start the patient on oxygen earlier and insist that the provider do more for the patient. Describe how you would apply the knowledge and skills that you obtained in Olivia Jones’s case to an actual patient care situation. I would feel much more competent in dealing with a patient who may develop eclampsia and know how to prevent seizures.
