Avoid smoking and alcohol,low sodium diet

diabetes, CHD, HTN, cardiomyopathy, sleep apnea, age, excessive alcohol

Fall precautions Monitor anticoagulation therapy (PT/INR)

PCP, cardiologist, possible home health

bleeding risks, medications, signs and symptoms

Anticoagulant medications, antiarrythmic medications

Anticoagulation therapy

Stoke, HF, bleeding risks, transient ischemic attack
