Opening Questions How did the simulated experience of Olivia Jones’s case make you feel?

Th e simulation made me feel a little anxious but I feel much more confident in my knowledge now because I got to apply what I have been learning in class.

Describe the actions you felt went well in this scenario.

I think my assessment of the patient and applying the knowledge of the recognizing critical symptoms of pre -eclampsia. I also thought my patient teaching went well too. A patient will be more calm when the y feel like they have a better understanding of what is happening and why certain things are being done and that can promote a better patient outcome.

Scenario Analysis Questions* EBP What are the indications that Olivia Jones’s preeclampsia has progressed?

Her blood pressure shot up to 171/102 mmHg, her lungs were filled with fluid so her kidneys were not filte ring correctly, and she had severe headache with nausea. Her bra in was beginning to swell and that could cause much bigger complications.

PCC What should the priority teaching for Olivia Jones include?

The Priority teaching should include expected affects of the magnesium sulfate and the danger signs of toxicity so they nurse can catch it as soon as possible. Also patient safety and minimizing outside stimulation is another priority.

PCC/I Olivia Jones was placed on magnesium sulfate for treatment of her preeclampsia. What are the indications for this drug, and what should be taught to the patient regarding side effects?

Magnesium sulfate is a mineral naturally found within the body and when administerd it is used to prevent seizures in pregnant women with conditions like yours, pre-eclampsia. If you feel very

weak, like you might pass out, flushing or difficulty breathing then call the nurse station immediately.

T&C What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format.

Olivia Jones is a 23-year-old African-American female, G1P0 at 36

weeks of gestation. She has been diagnosed with severe preeclampsia

and is admitted to the labor and delivery unit for assessment and

surveillance is complaining of a severe headache and nausea. She

has 3+ pitting edema and her blood pressure is 171/103 mmHG. There

are crackles at the base of the lungs. An IV with lactated Ringer’s is

running at 125 ml/hr and I just administered Magnesium Sulfate. Protein

dipstick is +4. Patient has been taught about safety and I reassessed for

toxicity and improvement about 30 minutes after giving her the

medication.

S What safety measures should be initiated while Olivia Jones is in the hospital?

The patient should be identified using 2 ways to check. Allergies should be assessed. Se izure pads should be put on the bed, have suctioning nearby and epinephrine in case of a seizure.

S/QI Based on your experience with Olivia Jones’s case, reflect on possible nursing actions for enhanced safety and quality improvement.

Patient teaching to the mother of the patient is very important because the patient may feel too sick to be able to recognize w hat is happening and doesn’t alert anyone. Also not watching TV or playing on the phone, dimmed lighting and rest should be main priority to keep the risk of eclampsia as low as possible.

Concluding Questions Reflecting on Olivia Jones’s case, were there any actions you would do differently? Explain.

Yes, I would have called the provider as soon as her blood pressure got so high because there were no medications in the mar for wht to give for her blood pressure and she also might have needed lasiks to remove the fluid from her lungs. Her oxygen levels were on the low side and that doesn’t provide for best oxygenation to the fetus.

Describe how you would apply the knowledge and skills that you obtained in Olivia Jones’s case to an actual patient care situation.
