Opening Questions

How did the simulated experience of Olivia Jones's case make you feel? Describe the actions you felt went well in this scenario.

I feel that after performing this Virtual simulation I am more aware of what to do when there is pre-eclampsia present. When in this situation I learned how to prioritize and what to do first. At first, I felt nervous but quickly realized what the simulation wanted me to do. I believe the scenario went well and everything was successful.

Scenario Analysis Questions EBP What are the indications that Olivia Jones's preeclampsia has progressed?*

Indications that the pre-eclampsia had progressed was that her labs showed changes and also her vital signs. There was also the presence of crackles in her lungs and severe pain.

PCC What should the priority teaching for Olivia Jones include? The primary teaching would be for her current problem and what she can do to recover. She should also be educated on the symptoms of her problem and or any medications that were administered.

PCC/I Olivia Jones was placed on magnesium sulfate for treatment of her preeclampsia. What are the indications for this drug, and what should be taught to the patient regarding side effects? This medication can help in preventing eclampsia which would cause her to have a seizure. This can be prevented with the proper care and medication that she received. Side effects of this drug can be GI upset, dysrhythmia, nausea which she displayed offten, and even vomiting.

T&C What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format.

During my hand off I would include all her vital signs and any problems that are currently being faced so that the nurse is aware. I would also report on any tests that were done and the results.

S What safety measures should be initiated while Olivia Jones is in the hospital? Safety precautions would be seizure pads put into place just in case a seizure occurred due to eclampsia. Also doing comfort measures such as darkening the room to decrease any type of overstimulation to the patient.

S/QI Based on your experience with Olivia Jones's case, reflect on possible nursing actions for enhanced safety and quality improvement. Actions that can be done are comfort measures of supporting the patient and darkening the room, not allowing a lot of people in the room with her, and having the appropriate equipment to aid this patient.

Concluding Questions

Reflecting on Olivia Jones's case, were there any actions you would do differently? Explain. Describe how you would apply the knowledge and skills that you obtained in Olivia Jones's case to an actual patient care situation.

Something I would have done differently is more comfort measures for this patient. I would have also administered medication for her nausea more frequently since it was on a PRN basis. I only administered it once at the beginning of the simulation and she was still complaining of severe nausea.

I would be more focused on treating her nausea and trying to prevent preeclampsia. Having the provider called in when the symptoms got worse would have been a wise decision. Since she had increasing symptoms I should have called for interprofessional help to ease her discomfort.
