D222 Video Health Assessment Success Guide

Tips/FAQs for the Video Task

The key to your success on this task is to read and follow the directions. Remember, you are a

professional nurse. As such, high levels of professionalism are expected. Reach out to your

instructor with any questions before you record.

Content & Equipment

Q1: What do I need to submit for evaluation? A. All items must be submitted together or your task will be returned without evaluation! 1. One consent form for your volunteer patient (they must be able to provide consent in your home state and be at least 18 years old). Please obtain a volunteer who is willing to sign and print their full name and complete all sections of the consent form prior to recording your video. All blanks on the consent must be completed, including printed full name, date, full address with city/state/zip, date of birth/age, and a handwritten signature. You will need to print the consent form, fill it out completely, and have your patient sign it before recording. Scan it to your computer to upload. You can also take a very clear digital photo of the form and upload the digital file. Suggested file types include PDF or Jpeg files. The entire form must be legible in your scan/image (full-length page with all wording visible). Click here to view a sample consent with all fields complete. 2. PAGE 2 of the consent form file is a required TIME LOG that tracks the hours spent on Shadow Health and the video assessment. You must fill out the dates each were completed (Shadow Health MUST be completed before your video recording) and sign the log. This must have the student’s handwritten signature. Click here to view a sample time log (located on page 2 of the consent). 3. URL of your Panopto video (Viewer Link). Your video URL link will be emailed to you by Panopto to your WGU email if you are using the downloaded Panopto version. (Note: Panopto capture does not email the Panopto links). Complete documentation (1–1 pages) of the Health Promotion activities, goals, and two journal articles

Any revisions submitted must contain all items as if the submission was brand new.

Q2: What supplies/equipment do I need for this assessment? A. Props are not allowed for this task. You must have a working stethoscope, a penlight or flashlight, watch or timer, BP cuff (manual or automatic), reflex hammer, cotton swab or pen cap for sensation, and tongue blade.

B. Not needed for the video: Snellen chart, otoscope or ophthalmoscope, tuning fork.

Head-to-Toe Assessment

Q3: How detailed should my assessment be? A. Follow the required assessment techniques shown on the Physical Assessment Skills List. Prior to beginning your video, review your materials and required techniques. Set up your area with all necessary equipment before recording. Ensure you do not block the camera’s view and that you speak loudly and clearly. The lighting should be adequate enough to view all techniques without difficulty.

Q4: What order or techniques are required in the head-to-toe video? A. You will follow the order of the “D222 Physical Assessment Skills List.” The assessment is split into multiple sections on the rubric (see sections for required minimum scores), as well as the requirement for the health promotion component. Use the words on the form to assist the evaluator in following you. For example, “now I’m going to assess your apical pulse for 60 seconds...” However, if you forget a step, you can do it at the end of your video (out of the normal order).

Q5: Can I use my child as my client in the video if they are a minor? A. No. All participants appearing in the video must be at least 18 years old to legally sign the consent form.

Q6: What resources can I use for this task? A. The Jarvis textbook is your best resource to review all of the techniques required for this assessment. Reach out to your instructor with specific questions and review the content in the COS thoroughly. Please note, cueing or receiving assessment directions from the patient or any other party is not allowed and is considered an academic authenticity (originality) concern. Please ensure your volunteer patient is instructed beforehand to not assist with the assessment..

Q7: How should I dress for the assessment recording? A. You must dress and communicate professionally for the recording. If your dress, communication, or both are deemed unprofessional, the entire video will be returned without evaluation, and you will need to rerecord and submit all four sections of the Health Assessment video. Scrubs are recommended but not required. Other acceptable professional dress options may include slacks, business tops, jeans (no holes and not torn), plain shirts, navy or black leggings, or button-up shirts. B. Unacceptable professional dress options may include, but are not limited to: pajamas, tank tops, brightly colored/patterned leggings, holey jeans or shirts, midriffs showing, shorts, slippers, or graphic t-shirts. (Note: your patient can wear any clothing item, so long as they are comfortable and their privacy is maintained). Unprofessional communication during the video demonstration includes the use of profanity, racial comments or slurs, vulgar statements, and any other verbal/non-verbal gestures or comments deemed inappropriate.

Q8: Does my patient need to take his/her shirt off for the Respiratory/Cardiac/Abdominal exam? A. No, it’s recommended your patient wear a tank top/t-shirt and shorts and be barefoot for lower extremity assessment. All assessments can be performed over the clothes for the comfort of the patient, so long as the evaluator is able to reasonably depict the appropriate landmarks for placement/positioning.

Q14. What do I need to do in preparation for recording? A. Review your materials and the assessment techniques that you will be required to perform. Set up your area. You should have all of your equipment ready to go before starting the recording. Do a test video to ensure the audio and lighting is appropriate. You may move your camera during the recording as needed. Be sure you do not block the camera’s view. Turn the camera or your patient as needed to ensure the assessment technique is visible in the frame. Speak loudly and clearly.  Success tip! Complete a test video before recording the assessment! This will allow you to review the audio and video images to ensure everything is in the frame and the audio is clear.

Q15: Will my video be available for anyone to see? A. No, the default setting is Private for every student video. Only WGU Evaluators will be watching your video. They are nursing faculty and employees of WGU.

Q16: What if I have issues with Panopto? A. Assessment Services (assessmentservices@wgu) is available for technical assistance with Panopto. Panopto Video Uploading How do I upload a video file into Panopto (.mp4, .wmv, .mov, etc.)? Note: This only applies if you used an alternative recording method and now have an external video file to upload into Panopto.

Required Components for Submission

Q17. What do I need to submit for evaluation of the task? A. Each time you submit, you must include all required components: signed patient consent (dated within the last month), signed time log documenting hours completed, the Panopto URL, and health promotion documentation. Even if you have previously submitted these items, they must all be submitted together every time you submit for evaluation.

Q18: How do I get my video from Panopto into the submission for evaluation? A. Copy your video viewer link/URL from Panopto and paste it into the URL link box as indicated in the submission area of the task. For assistance, review the specific FAQ article Here.
