Introductory Paragraph The data for the following section on Caldwell County, NC was obtained from the County Health Rankings and Roadmaps website (County Health Rankings and Roadmap, n.). The data collected was reviewed and compared to that of the state of North Carolina and nationally with the United States statistics. As a resident of Caldwell County, it was interesting to look at how the county’s overall health compares to the state and national levels on certain health disparities in our area. The seven health disparities that were chosen for review were the incidence of premature deaths, deaths related to alcohol-impaired driving, the rate of sexually transmitted diseases, the rate of violent crimes, children living in poverty, air pollution, and the

unemployment rate. These were chosen for their capacity for great change in Caldwell County to create a healthier environment for the residents and reduce health disparity.

Discussion of Health Trends

Caldwell County ranks 68th out of 100 counties in North Carolina for the overall health outcome ranking with a 9,600 length of life rate (years of potential life lost before age 75) which is higher than the state and top U. Performers rates. The rate of premature deaths has increased from 8,800 in 1998 to 10,400 (Years of Potential Life Lost Rate) in 2019. (County Health Rankings and Roadmap, n.). This statistic shows that Caldwell County, NC is in the lower bracket for good health in the state and nationally. During the review of secondary data for the county, state, and nation, it became clearer that several health disparities are important to address for the overall health of the county.

Analysis of data regarding the rates of alcohol/impaired driving deaths in Caldwell County shows that while Caldwell County reached an all-time high in 2016 of 60%, the county had decreased to no impaired driving deaths in 2020. This figure is considerably lower than the state average of 23% of vehicle accidents involving alcohol or impairment and the national average of 26%. (County Health Rankings & Roadmaps, n.). This trend has improved in Caldwell County which has a positive outcome for the health of the population.

The next trend that was reviewed was the rates of sexually transmitted diseases in Caldwell County. While the county rate for sexually transmitted diseases remains significantly lower than the state rate of 669 per 100,000 people and the U. rate of

Roadmaps, n.). Currently, in 2022, the unemployment rate in Caldwell County is more than twice the national average of 3% at 7%. (U. Bureau of Labor Statistics, 2022). This ranking produces concern for the population of Caldwell County and if left unattended, the overall health of the county would suffer.

Air Pollution (Average Daily Density of AirPollutants) 8 7 5. Percent with Severe Housing Problems 13% 15% 9% County Health Rankings & Roadmaps. n.: Caldwell County; North Carolina; United States Comparison of Data In the table above, the data was obtained by using the County Health Rankings & Roadmaps website to obtain information on some factors that influence the health of the population. The table shows the comparison of Caldwell County, North Carolina, and the Top U. Performers and the trends that can arise when comparing local, state, and national information. (County Health Rankings & Roadmaps. n.). By analyzing this type of information, the goal is to bring to light the health disparities in the community that need to be addressed, identify stakeholders, and create an action plan that would improve the overall health of the community. While reviewing the data for Caldwell County, a few categories stood out as problem areas for the county. While reviewing the data, it is noted that socioeconomically, Caldwell County is below average in comparison to North Carolina and the top performers. This is made evident by comparing first the unemployment rate of 7% for Caldwell County and 7% in North Carolina to the U. Top Performer rate of 4%. This shows a double in the unemployment in this area, leading to the second area of concern for Caldwell County, which is a 13% rate of the population with significant housing issues. While this is a 2% decrease in North Carolina’s rate at 15%, if compared to the top performer’s rate of 9%, Caldwell County’s rate is 4% higher. The socioeconomic problems escalate and build upon themselves with higher than the top performer averages of children living in single-parent homes and children eligible for free or reduced meals. (County Health Rankings & Roadmaps. n).

To contrast the negative data in the graph above, Caldwell County is lower in the rate of violent crimes at 169 per 100,000 people compared to the North Carolina rate of 351 per 100,000 people. While this remains significantly higher than the top performer rate of 63 per 100,000 people, this rate does show that Caldwell County has some type of plan in place to reduce this rate in comparison to the state. Overall, the data in the above table shows that Caldwell County has many factors that they need to address and improve upon for better community health. (County Health Rankings & Roadmaps. n). Summary

Significant Findings While reviewing the data from the Census Bureau and the County Health Rankings & Roadmaps websites, it was noted that the socioeconomic situation in Caldwell County is of great concern. While completing the analysis, I noted that the unemployment rate is 7% for Caldwell County. (County Health Rankings & Roadmaps. n). This rate is two times higher than that of the United States unemployment average of 3%. (U. Bureau of Labor Statistics, 2022). Because unemployment can impact numerous other areas of life, unemployment is the main trend that I find to have the greatest potential to be positively affected by the creation of an action plan for Caldwell County, North Carolina. By decreasing the unemployment rate in Caldwell County, the potential for improved health and health outcomes improves. Action Plan The first action in preparation for creating an action plan is to identify the main stakeholders who would potentially benefit the most from addressing the unemployment rate in Caldwell County. After the team of stakeholders has been identified and organized,

tax credits for moving into the community. Implementing these tax credit incentives to companies would increase the number of available jobs in the community and offer the opportunity for those who are unemployed to work in their communities among their peers. With the enlistment of local officials and national companies, the rate of unemployment in Caldwell County could be impacted for the good and decrease the health disparities that arise from high unemployment rates.

Raise Public Awareness and Promote Public Engagement

The Advanced Professional Nurse (APN) uses technology in various ways to integrate informatics and raise public awareness of health disparities and inform the public of services and help available to them. For the action plan listed above for Caldwell County’s high unemployment rate, the use of three common informatics, social media, free online classes or webinars, and a mobile app to help with job searches in the county and deliver relative evidence-based information on services and programs that are available. First, social media has become a great tool for APNs to use daily in recruitment efforts. For Caldwell County’s action plan, APNs could use social media such as LinkedIn, Indeed, or Facebook to post job openings in their places of employment and review candidates’ resumes, and set up interviews. This would help to reduce the unemployment rate in Caldwell County by increasing the exposure to current job openings and those who are looking for work. Social media also provides a great platform for data to be delivered to a diverse group of individuals in extended areas.

The second form of technology that would be an asset to Caldwell County to reduce the unemployment rate would be to implement free online classes and webinars to provide and promote continuing education and job training. Providing free education,

empowers individuals to set goals and reach them by giving them tools they can use to advance their careers and gain employment. This would be a great step in decreasing the effects of unemployment-related health disparities.

Lastly, create a mobile app that could easily be installed onto your mobile device that offered notifications of job openings in real-time. Placing access to thousands of employment opportunities in people’s hands allows them to be on guard at all times for opportunities to gain employment as well as provide evidence-based data to help facilitate good interviews and successful job searches. Mobile apps are often utilized more than websites because of ease of access and use. The APN could create the app and create within the app settings for types of jobs, monetary requirements, and job location needs. Using this app could increase awareness of job prospects that may have been otherwise overlooked.

Monitoring and Evaluating Action Plan To monitor and keep track of the use of each of the listed informatics, the Advanced Professional Nurse (APN) could integrate the use of surveys and polls for individuals who frequent the social media pages or websites to fill out. To provide monitoring of the mobile app use, the creator would create within the app a system to track users based on different characteristics such as age, race, and age. Once the data statistics from these various forms are obtained, they can be used to alter the websites and apps to reflect the needs of those using them. The data can also be used to add more goals and advancements to the action plan that is in place for Caldwell County’s high unemployment rate. The data could be stored in an Excel spreadsheet to reflect the action plan’s effectiveness and show trends in the data by using various graphs and tables such as a

Secondary data was obtained from the U. Labor Statistics website at bls/bls/news-release/empsit.htm and the Workforce Solutions website at workforcesolutionsrca/locations/caldwell to obtain supporting data that was used to substantiate the main disparity for this essay and create an action plan based off of that secondary data.
