D222: Comprehensive Health Assessment Victoria Dusza College of Health Professions, Western Governors University Dolores Northrup October 12, 2022

The two recommended health promotion activities based on the patient's health assessment will be to lose weight and increase metabolism to reduce constipation. During auscultation of the patient's stomach, I noted that the bowel sounds were decreased in all four quadrants. The patient's last bowel movement was three days ago, which indicates constipation. Upon examination, the patient stated that he doesn't consume five portions of fruit and vegetables per day, reported an unhealthy diet, and believes that his diet could be improved. Based on this finding patient received education about what kind of diet should be followed to increase metabolism and decrease constipation. The education included the following: incorporate fruits and vegetables into every meal, eat vegetables as a snack, add fruit to your cereal in the morning, and make sure at least half of all your grains are whole grains. Choose 100 percent whole-wheat bread, brown rice, and whole-grain cereal over refined grains like white bread, white rice, and sweetened cereals (Graham, 2022).

Additionally, based on the information provided by the patient, his height is 6 feet 1 inch, and his weight is 220 pounds, which makes his BMI 28. A BMI of 25–29 indicates that a person is slightly overweight. According to the National Library of Medicine, a high BMI is associated with a range of comorbidities, including diabetes, cardiovascular disease, obstructive sleep apnea, and cancer; however, modest weight loss in the 5%–10% range, and above, can significantly improve health related outcomes (Sharon, 2017). The patient was educated about‐ how physical exercise and a healthy diet can help to lose 5-10% of his weight.
